TINY TAILS RETREAT, LLC PARTNER PAYMENT & SERVICE AGREEMENT

This Agreement is entered into between Tiny Tails Retreat, LLC and the undersigned client above.

Tiny Tails Retreat provides pet transportation, veterinary visit support, in-home care visits, and related services as requested by the Client.

Client agrees to enroll in one of the following monthly partnership plans:

Starter Plan – $300/month (10 service credits)
Growth Plan – $500/month (20 service credits)
Payments are billed monthly on a recurring basis.
Payment is due at the time of enrollment and automatically renews each billing cycle unless canceled.

Each service booking utilizes one (1) credit unless otherwise specified.
Additional charges may apply for:
- Mileage beyond included range
- Additional pets
- Extended wait times
- Specialty or custom services
Credits reset monthly and do not roll over unless otherwise agreed in writing.

Client agrees to pay any applicable additional charges incurred at the time of service or invoiced separately.

Payments are non-refundable once processed.
Client may cancel their plan at any time; however:
- Cancellation will take effect at the end of the current billing cycle.
- No prorated refunds will be issued.
Service cancellations:
- 4+ hours notice: eligible for rescheduling or credit return
- Less than 4 hours / no-show: credit forfeited

Client authorizes Tiny Tails Retreat to charge the payment method on file for:

Tiny Tails Retreat agrees to provide safe, compassionate handling of all pets.Client acknowledges:

Client agrees to provide accurate pet and service information at all times.

To the fullest extent permitted by law, Tiny Tails Retreat shall not be liable for:
