AGREEMENT & CONSENT
I authorize Tiny Tails Retreat LLC to provide pet transport, care visits, veterinary visit support, and related non-medical pet support services for my pet(s).
By signing this agreement, I confirm that I am the legal owner or authorized caregiver of the pet(s) listed on my account and that I have authority to approve services on their behalf.
This agreement applies to all current and future services scheduled with Tiny Tails Retreat LLC unless replaced by a newer written agreement.
By signing this agreement, I acknowledge and agree to all terms below.
GENERAL LIABILITY WAIVER & ASSUMPTION OF RISK
I understand that working with animals involves inherent risks, including but not limited to injury, illness, stress, anxiety, escape, bites, scratches, property damage, accidents, vehicle-related risks, and unexpected medical events.
I agree that:
• My pet(s) are current on required vaccinations and safe to handle to the best of my knowledge.
• I have disclosed all known medical conditions, behavioral concerns, anxiety, aggression, escape tendencies, bite history, allergies, medications, and special needs.
• Tiny Tails Retreat LLC provides non-medical pet support services only.
• Tiny Tails Retreat LLC does not diagnose, treat, prescribe, or provide veterinary medical care.
• Tiny Tails Retreat LLC will use reasonable care; however, I understand that accidents, illness, injury, escape, or other incidents may occur despite reasonable precautions.
• I release Tiny Tails Retreat LLC, its owners, employees, contractors, representatives, and agents from liability for injury, illness, death, escape, loss, or property damage except in cases of gross negligence or intentional misconduct.
• I agree to indemnify and hold harmless Tiny Tails Retreat LLC from claims, damages, expenses, veterinary bills, property damage, or injuries caused by my pet(s), including injury to people, other animals, property, vehicles, or third parties.
EMERGENCY VETERINARY AUTHORIZATION
If I cannot be reached in an emergency, I authorize Tiny Tails Retreat LLC to:
• Seek veterinary care for my pet(s).
• Transport my pet(s) to my listed veterinarian or the nearest available emergency veterinary facility.
• Communicate with veterinary staff on my behalf.
• Provide consent for reasonable emergency care when immediate action is needed for the safety or welfare of my pet.
I understand that all veterinary charges are my responsibility and are to be paid directly by me, the pet parent, or responsible party.
SERVICE CONSENT
This agreement applies to any services I schedule, including:
• Pet Transport / Shuttle Services
• Veterinary Visit Support, including transport and onsite assistance
• In-Home / Customized Care Visits
• Feeding, water refresh, walks, companionship, wellness check-ins, and basic care support
• Other related pet support services requested and accepted by Tiny Tails Retreat LLC
Tiny Tails Retreat LLC reserves the right to refuse, stop, or modify services if a pet, home, environment, or situation is unsafe.
TRANSPORT TERMS
For transport services, I understand and agree that:
• Pets will be secured using a crate, carrier, harness, seat belt, leash, or other appropriate safety restraint.
• I am responsible for providing an appropriate carrier, leash, collar, harness, or restraint if requested.
• Travel times may vary due to traffic, weather, appointment delays, pet behavior, or scheduling changes.
• I will provide accurate pickup and drop-off instructions.
• Tiny Tails Retreat LLC is not responsible for delays caused by traffic, weather, veterinary office delays, road conditions, emergencies, or circumstances outside its control.
• I understand transport may create stress or anxiety for some pets, and I accept this risk.
VETERINARY VISIT SUPPORT TERMS
For veterinary visit support, I understand and agree that:
• Tiny Tails Retreat LLC may transport my pet to and from the veterinary office.
• Tiny Tails Retreat LLC may remain onsite during the appointment when scheduled as part
of the service.
• Tiny Tails Retreat LLC may communicate information between me and veterinary staff.
• Tiny Tails Retreat LLC is not a veterinary provider and does not diagnose, treat, prescribe, interpret test results, or make medical decisions except as authorized in an emergency.
• Tiny Tails Retreat LLC is not responsible for veterinary recommendations, medical outcomes, treatment results, diagnosis, medication decisions, or fees charged by the veterinary office.
• All veterinary charges are paid directly to the veterinary office by the pet parent or responsible party.
CARE VISIT TERMS
For care visits, I understand and agree that services may include feeding, fresh water, walks, companionship, basic cleanup related to pet care, wellness check-ins, and other agreed-upon non-medical support.
I agree that:
• I will provide clear written instructions for feeding, medications, routines, access, and special needs.
• Medication assistance is limited to owner-provided, non-invasive instructions.
• Tiny Tails Retreat LLC does not administer injections or perform medical procedures.
• Tiny Tails Retreat LLC is not responsible for complications from medications, missed doses due to inaccurate instructions, inaccessible medication, pet refusal, or unsafe conditions.
• Tiny Tails Retreat LLC is not responsible for household conditions outside the agreed pet care scope.
HOME ACCESS TERMS
If home access is required, I agree to provide safe, accurate, and reliable access to my home.
I understand and agree that:
• I am responsible for providing working keys, codes, lockbox access, gate access, alarm instructions, and parking instructions.
• Tiny Tails Retreat LLC is not responsible for service delays or missed visits caused by incorrect access information, locked gates, non-working keys, alarm issues, or inaccessible property.
• I authorize Tiny Tails Retreat LLC to enter my home for scheduled services.
• I confirm that my home, yard, fencing, gates, doors, and access areas are safe and secure.
•I authorize Tiny Tails Retreat, LLC to communicate with veterinary staff on my behalf.
I understand that all veterinary charges are my responsibility and are to be paid directly by me, the pet parent, or responsible party.
• Tiny Tails Retreat LLC is not responsible for pre-existing home damage, property conditions, faulty locks, broken fences, open gates, malfunctioning doors, or unsafe areas.
• I agree to notify Tiny Tails Retreat LLC of anyone else who may enter the home during service times, including family, neighbors, cleaners, contractors, or other pet care providers.
PET BEHAVIOR DISCLOSURE
I confirm that I have disclosed any history of:
• Aggression
• Biting or scratching
• Anxiety or fear
• Escape attempts
• Leash reactivity
• Medical concerns
• Special handling needs
• Issues with other animals or people
I understand failure to disclose behavior, medical conditions, or safety concerns may result in service refusal, cancellation, additional fees, or termination of services.
I am responsible for any injury, damage, claim, cost, or expense caused by my pet’s behavior.
SCHEDULING, PAYMENTS & POLICIES
I acknowledge that all services are subject to the current policies of Tiny Tails Retreat LLC.

I agree to review and abide by the current policies prior to each booking.
In the event of a conflict between this agreement and the policies listed on tinytailsretreat.co, the website policies shall govern unless prohibited by law.
PHOTO & MEDIA RELEASE
Tiny Tails Retreat LLC will not knowingly share my personal address or private identifying information in marketing materials.
COMMUNICATION CONSENT
I agree to receive service-related communication by phone, text message, and/or email, including scheduling updates, appointment confirmations, service updates, and follow-up communication.
I understand I may opt out of text messages at any time by replying STOP, but opting out may limit service-related communication.
SIGNATURE
By signing below, I confirm that I have read, understand, and agree to all terms outlined in this agreement.
